WELCOME TO THE HEALTH OFFICE
The Scandia Elementary Health Office is directed by Christine Smith, LPN. It is the first aid station for ill or injured children during school hours. Nurse Christine will assess whether or not the student will be sent home and will contact the family accordingly. If you have any questions, please call the Health Office at 982-3304.
Please visit the Forest Lake Area School district website,
Health Services, for more information.
ATTENDANCE
Attendance is tracked through the Health Office as well. Please call the
Attendance Hotline (available 24 hours a day) at 982-3399 if you know your child will be absent. If you need to pick up your child early for a doctor or dental appointment please call the Main Office at 982-3300 so we can contact the classroom teacher.
IMMUNIZATION FORM
MDH Student Immunization form
EMERGENCY FORMS
Emergency forms are necessary for each child. If a child becomes ill or injured in school, parents will be contacted first. If the school is unable to contact the parents, the alternate person on the emergency form will be called. Please notify the school when changes in telephone numbers and/or alternate persons occur during the year.
MEDICATION
Forest Lake Area Schools has a medication policy which states that NO MEDICATIONS (including over-the-counter medication such as Tylenol or Advil) will be given in school without a written order from the doctor and parent written permission. Medication authorization forms may be obtained from the school office or doctors clinic. The medication must be in a current prescription bottle or original bottle for over-the-counter medication. Parents are requested to bring medication to school rather than sending it with their child.
TOO SICK FOR SCHOOL?
HANDWASHING is the single most important way to keep from getting sick. Other measures include proper diet, adequate rest, covering a cough, and avoiding large crowds of people.
GENERAL GUIDELINES FOR EXCLUSION FOR ILLNESS:
It is important for each and every one of us to follow the same guidelines when deciding whether or not a child should be excluded from school. The same guidelines apply to adults. If you have questions about these guidelines or help in deciding if your child should stay home from school, contact the school nurse at 651-982-3304, or one of the district nurses. By following these guidelines, the risk to all students can be kept to a minimum.
- If a child has had a fever of 100 degrees or more, the child should stay home for 24 hours after the temperature returns to normal.
- If a child has vomited or had diarrhea, the child should stay home until 24 hours after the last episode.
- If a child has any rash that may disease-related or you do not know the cause, check with your family physician before sending the student to school.
- If a child is being treated for strep, exclude from school until 24 hours after antibiotic treatment is started and until the child is feeling well enough to be in class.
- If a child has symptoms of influenza (chills, body aches, headache, fever, sore throat, followed by cough, runny nose, and possibly stomach ache) exclude from school until well, usually 2 to 7 days.
- If a child has pinkeye (conjunctivitis) with pus and/or fever or eye pain. An exam by a Medical Doctor is recommended.
